Job Summary The Nurse Practitioner provides primary care and treatment to children referred to the University Developmental Assessment Clinics ( UDAC ), working primarily with the Neonatal Follow-up Program, Child Development Program, and Heart Center Neurodevelopmental Program performing in person or virtual clinical visits. Performs assessments and provides recommendations for interventions as necessary to enhance and improve developmental outcomes. Clinics are held in Salt Lake City and Lehi. Responsibilities
Essential Functions 1. Assess the physical and developmental condition of patients referred to the UDAC by performing physical / developmental examinations and obtaining medical histories. Formulate the medical and developmental plan based on patient’s condition. 2. Collaborate with hospitals, school districts, and community agencies. Prepare reports for parents, primary care providers and other agencies. 3. Consult with physician(s) and multidisciplinary team regarding patient health care plans and treatment. Work in conjunction with the patient’s primary care provider or medical home. Participate in care conferences and staffing meetings. 4. Order, interpret, and evaluate diagnostic tests to identify and assess the patient’s clinical problems and health care needs. 5. Initiate and/or recommend medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy. 6. Comply with medical records policies regarding the complete and accurate documentation of patient care. Initiate and maintain required records and legal documents. 7. Participate in quality improvement activities, processes and provider meetings. 8. Identify and facilitate access to other behavioral, medical or community resources and follow up as necessary. 9. Work closely with the UDAC team, providing guidance and oversight as necessary. 10. Evaluate, plan, implement, and re-evaluate the care of individuals as required. 11. Train and supervise students including pre and post graduate level trainees 12. Participate in research ongoing with the UDAC as required. 13. Perform other related duties as assigned. The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient’s status and provide care as described in the department’s policies and procedures manual.
Minimum Qualifications
Required Licensure to practice as a Nurse Practitioner in the State of Utah. Preferred Certification in ACLS / BLS / NRP / ATLS may be required by your department or by privileging criteria. Utah State prescriptive privileges. Licensure is obtained by completing a Master’s degree in Nursing or equivalency (the only equivalency to the Master’s degree is current licensure as a Nurse Practitioner) and a National Certification which requires the successful completion of a nationally accredited examination. UHPPM 1-56 requires that newly hired non-physician providers apply for credentialing and privileging at the time of hire. This position is patient-sensitive and must fulfill all associated requirements. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.
