Nurse Practitioner – High Risk (Sherman Oaks, CA)

Vivo HealthStaff is recruiting for a Nurse Practitioner – High Risk in Sherman Oaks, California. This is a permanent position with salary and benefits.

About the Opportunity
Our client, a leading healthcare provider in Southern California, is seeking a dedicated Nurse Practitioner to provide high-quality care to high-risk patients in the San Fernando Valley region. This position offers the opportunity to make a meaningful impact by managing post-discharge care for patients with complex conditions, including Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD), through home and clinic-based services.

Key Responsibilities

  • Provide comprehensive care to high-risk patients in home and clinic settings.
  • Follow evidence-based clinical guidelines to ensure quality patient care.
  • Document all patient interactions and care provided in accordance with regulatory standards.
  • Coordinate follow-up care, including access to social and behavioral health services.
  • Monitor and document clinical and social issues for patients.
  • Conduct all visits in a timely manner, adhering to group and health plan protocols.
  • Perform additional duties as assigned.

Qualifications

  • Current certification as a Nurse Practitioner.
  • Minimum of 3-4 years in healthcare, with specific experience managing COPD and CHF patients.
  • Experience with high-risk, complex patient populations preferred.
  • Home care experience is a plus.
  • Excellent verbal and written communication skills in English (bilingual in Spanish preferred but not required).
  • Proficiency in Microsoft Office applications.
  • Ability to work independently while meeting team deadlines.

Compensation and Benefits

  • Salary: $140,000 – $160,000 annually, based on qualifications, market location, experience, and skills.
