Pay: Base salary + uncapped productivity bonus paid monthly

Position Summary
We’re partnering with a well-established post-acute care group seeking compassionate, self-driven Nurse Practitioners to deliver high-quality care in Skilled Nursing Facilities (SNFs) across Indianapolis. Whether you’re an experienced provider or a recent grad, this role offers strong clinical support, structured onboarding, and ongoing mentorship to help you thrive. Responsibilities
Perform assessments, diagnosis, and infectious disease treatment
Collaborate with physicians and SNF staff on care planning
Develop weekly rounding schedules based on facility needs
Maintain timely, accurate EMR documentation
✅ Qualifications
Active, unrestricted Indiana Nurse Practitioner license
Master’s degree in Nursing (MSN) from an accredited program
Minimum 1 year experience in SNF or long-term care (2+ years preferred)
Excellent interpersonal and clinical decision-making skills
Able to work independently in a fast-paced setting
